Total Hockey Academy

This form will need to be filled out by individuals coming to Total Hockey Academy for the first time for a Flex Pass Training Session, Team Training or any other Camp offered at Total Hockey Academy. This Form must be signed and filled out before training can begin. 

Please print out form, fill out the appropriate information, and bring it with you when you come to Total Hockey Academy.
